Quick Phonics Screener Assessment

Need a quick, reliable way to assess your students’ phonics skills? From letter naming to multi-syllabic words, this resource has you covered!

Phonics screening and assessment is essential to knowing where your students are in their growth as readers and for planning effective instruction.

This set of phonics screeners includes decoding assessments and progress monitoring that you can use all year long and in multiple grade levels. With nonsense words and sentences, too, it’s an easy way to track and document student growth!

Includes:

  • Teacher instructions
  • Student-facing assessment sheets
  • Teacher data collection recording sheets
  • Phonics pattern cards for remediation and instruction

These screeners can be used from kindergarten on up and cover the following phonics skills:

  • letter naming
  • letter sounds
  • CVC words
  • digraphs
  • blends
  • CVCe words
  • r-controlled vowels
  • vowel teams and diphthongs
  • soft C and G
  • silent consonants
  • 2-syllable words
  • 3-syllable words
  • 4-syllable words

Each assessment after letter names/sounds includes:

  • real words
  • nonsense words
  • real words in context (sentence reading)

The progress monitoring sheets include 3 administrations for each phonics skills with space for student name, date, and score. The minimum proficiency score for each section is provided.

In addition to the phonics screeners and progress monitoring sheets, there are letter cards for every phonics pattern that can be used for word building and phoneme-grapheme mapping activities.

I designed these phonics assessments based on my years of experience as a reading intervention teacher. They are a great tool that will help you:

  • plan instruction
  • identify phonics deficiencies
  • plan reading interventions
  • track and document RTI
  • monitor progress toward IEP goals
  • inform parent-teacher conferences

If you’re a homeschool parent, these screeners are a great tool for pinpointing where to focus your phonics lessons. If your child is struggling to read on level, a comprehensive phonics assessment can help you identify where the problem lies.
